Whole person care involves the patients’ physical and mental health including, medical, social, behavioral, employment, housing, nutrition, and community needs. We often treat these items separately, forcing patients to go to multiple providers to get the care they need. Considering the quadruple aim we have invited panelists to talk about the integration of care and what is happening on the ground. As we take a peek into the past, we will unravel the work that is being done now to join these two worlds together. The audience will gain perspective on new initiatives taking place to interoperate our healthcare system into one integrated pathway to quality care.
